SECOND CHANCE, FIRST PITCH RAFFLE

Who may participate: The Chicago Cubs Charities Second Chance, First Pitch Raffle (the “Raffle”) is open to participants who are 18 years of age or older and purchase Chicago Cubs Charities 50/50 Raffle Ticket valued at $20 or more during a Chicago Cubs regular season home game. Employees, officers, and directors (including immediate family members and members of the same household) of Chicago Cubs Charities (“Sponsor”), Chicago Cubs Baseball Club, LLC, Wrigley Field Enterprises, LLC, Levy Premium Food Service LP, Levy Restaurants and anyone who works at Wrigley Field or for Cubs or any related party (collectively, Sponsor and all other entities are the “Released Parties”) are not eligible to participate in the Raffle or win a prize.
How to enter: During the 2013 regular season, purchase a Chicago Cubs Charities 50/50 Raffle Ticket valued at $20 or more to receive an entry form:
Chicago Cubs Charities 50/50 Raffle Tickets will be sold during each Chicago Cubs regular season home game during the 2013 regular season. Tickets may be purchased through the second out of the bottom of the sixth inning from uniformed Chicago Cubs Charities raffle ticket sellers located throughout the park and in the vicinity of Wrigley Field. In the event of a daytime double header, there will be one raffle and tickets will be sold through the second out of the bottom of the sixth inning of the second game, provided, however, in the event of a day-night double header, there will be two raffles and tickets will be sold through the second out of the bottom of the sixth inning of each game. Tickets are non-refundable.
Scan the QR code on the raffle ticket with your smart phone to enter. If you do not own a smart phone, fill out the entry form on the bottom of your ticket and place your completed entry form in one of two entry buckets:
The Second Chance First Pitch raffle entry bucket at the Chicago Cubs Charities 50/50 Raffle kiosk located on the Wrigley Field concourse during each regular season home game; or
Visit the Wrigley Field Administrative Office during a regular season home game or normal business hours and place your completed entry in the Second Chance First Pitch raffle bucket.
Drawing: From all eligible entries received, Sponsor will randomly draw one raffle ticket on or about September 25, 2013. The decisions of Sponsor are final and binding on all matters relating to this Sweepstakes. Winner need not be present at drawing.
PRIZES: One (1) Prize will be awarded. Winner will receive four (4) Field Box Tickets to a 2014 regular season home game at Wrigley Field and the opportunity to throw out one ceremonial first pitch. (Estimated Retail Value of tickets: $ 465.92). Exact Prize details, including game date and seat location, to be determined in the sole discretion of Sponsor. TRANSPORTATION TO/FROM THE GAME AND LODGING NOT INCLUDED.
Each Winner is fully responsible for any and all applicable federal, state and local taxes (including income and withholding taxes). All costs and expenses associated with Prize acceptance and use not specified herein as being provided, including but not limited to lodging, transportation costs, meals, gratuities and other expenses incurred by accepting the Prize are the sole responsibility of the Winner. Each Winner’s companions must be of legal age of majority in their jurisdictions of residence (and at least eighteen (18) years of age), unless accompanied by a parent or legal guardian. Major League Baseball game, event and exhibition dates and times are determined in the sole discretion of the Office of the Commissioner of Baseball and/or the applicable Major League Baseball Club, as applicable, and may be subject to change. The terms and conditions of the tickets awarded as part of any Prize will govern in the event a legal game, event or exhibition, as defined by Major League Baseball, is not played due to weather, an Act of Nature, an act of terrorism, civil disturbance or any other reason. Each Winner and his/her guest(s) agree to comply with all applicable ballpark regulations in connection with the Prize. Sponsor and the Chicago Cubs reserve the right to remove or to deny entry to any Winner and/or his/her guest(s) who engage(s) in a non-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person at the game, event or exhibition. Released Parties (defined below) will not be responsible for Acts of Nature, acts of terrorism, civil disturbances, work stoppage or any other natural disaster outside their control that may cause the cancellation or postponement of any Major League Baseball game, event or exhibition.
The Prize is non-transferable and non-assignable, with no cash redemptions except at Sponsor’s sole and absolute discretion. Sponsor reserves the right to substitute a prize (or portion thereof) with one of comparable or greater value at its sole and absolute discretion.
NOTIFICATION: Each potential Winner will be notified at the e-mail address, postal address and/or telephone number (in the sole discretion of Sponsor) provided at the time of entry (the "Prize Notification"). In the event that any potential Winner does not respond to any such Prize Notification within three (3) days of the date of issuance, a disqualification will result and prize will be forfeited and, at Sponsor’s sole discretion and time permitting, an alternate potential Winner may be randomly selected from among all remaining eligible entries. Each potential Winner may be required to submit his/her valid social security number and/or other identification to Sponsor and may be required to execute, have notarized and return an Affidavit of Eligibility and Release of Liability and, unless prohibited by law, Publicity, within five (5) days of date of issuance. Failure to submit any identification required by Sponsor or to return the required documents within the specified time period, noncompliance with these Official Rules or the return of any prize or Prize Notification as undeliverable may result in disqualification and prize forfeiture and, at Sponsor’s sole discretion and time permitting, may cause an alternate potential Winner to be randomly selected from among all remaining eligible entries. Neither Sponsor nor Released Parties is responsible for or shall have any liability whatsoever for lost or stolen tickets, tickets damaged to the point of illegibility, or tickets redeemed in error by its agents. Ticket will be void and prize shall not be paid/redeemed if there is proof the ticket was stolen, unissued, ineligible, altered, counterfeit in whole or in part, defective, printed in duplicate or printed/produced in error. Unauthorized persons copying, selling, or altering tickets are subject to prosecution. All costs and expenses associated with claiming the prize and any and all applicable taxes are the responsibility of the winner.
WAIVER OF LIABILITY/PUBLICITY RELEASE: By participating in the Raffle and submitting an entry, each entrant agrees to (i) be bound by these Official Rules, including all entry requirements, and (ii) waive any and all claims against the Sponsor, Chicago Cubs, Wrigley Field, Major League Baseball, the other Major League Clubs and each of their respective parents, affiliated companies, subsidiaries, officers, directors, employees, agents, licensees, distributors, dealers, retailers, printers, representatives and advertising and promotion agencies, and any and all other companies associated with the Raffle, and all of their respective officers, directors, employees, agents and representatives (collectively, “Released Parties”) for any injury, damage or loss that may occur, directly or indirectly, in whole or in part, from the participation in the Sweepstakes or from the receipt or use of any Prize or any travel or activity related to the receipt or use of any Prize. By entering this Sweepstakes, each entrant gives his/her express permission to be contacted by the Sponsor by telephone, e-mail and/or postal mail for Sweepstakes purposes. Each Winner, by acceptance of Prize, grants to Sponsor and each of its respective designees the right to publicize such Winner’s name, address (city and state of residence), photograph, voice, statements and/or other likeness and prize information for advertising, promotional and/or trade and/or any other purpose in any media or format now known or hereafter devised, throughout the world, in perpetuity, without limitation and without further compensation, consideration, permission or notification, unless prohibited by law. All tickets, transactions, participants and winners associated with the Sponsor 50/50 Raffle are subject to federal and State law, applicable City of Chicago ordinances, and these Official Rules and any lawful changes thereof.
